A Child Care Provider's Guide to the Child Care Assistance Program (CCAP) I have never been a child care provider through the program. Is there anything I need to do before I can start receiveing payments? Yes! Child Care Home Providers that are NOT LICENSED through the Illinois Department of Children and Family Services (DCFS) must submit: - copy (front and back) of a current picture identification (i.e. Illinois driver's license, Illinois state ID, or militaty ID) - copy of your social security card - completed W9 form - completed CANTS (Child Abuse and Neglect Tracking System) form Any providers that are LICENSED must submit: - W9 form - Child Care Rate Certification form How often will I get paid? Regular payments will come on a monthly basis. Please explain monthly co-payments. All parents are required to share in the cost of child care for their children. Parents MUST make a payment, called the Parent Co-Payment to their child care provider once per month. The amount of the parent co-payment is based on gross monthly income, family size and numer of children receiving child care. *** The monthly co-payment amount assigned to the parent is deducted from the provider's check. It is the child care provider's responsibility to collect the co-payment. What is the daily rate that I will be paid? Child Care Home Provider Rates

I have submitted my Child Care Certificate for payment. How Can I check on the status of my payment? You can call 1-800-804-3833. This toll free number is available 24 hours a day 7 days a week. You can also get payment information by visiting the state comptroller’s website at: www.comptroller.state.il.us and select "vendor payments". Are taxes taken out of a child care providers check? NO. Taxes cannot be taken out of the checks you receive. If you are paid at least $600 within a year, you will receive a 1099 form stating the child care income you earned for the year. Are you changing your address? Please notify us. We have a change of address form for you to complete. *If your address is not current, your payments may be delayed and you may not receive important notices.
